









What are the Ages & Stages Questionnaires?
The Ages & Stages Questionnaires is a screening system designed to detect developmental delays in children. There are
questionnaires designed to be given at specific ages. The questionnaires screen your child for gross motor skills, fine
motor, personal-social skills, problem solving, and communication.
How are the Questionnaires used?
1. Determine you child's "Corrected Age". For a child OVER 24 months old, use the actual age. For a child less than 24
months old, subtract any prematurity to the nearest month. For example, if your child was born 3 weeks early, subtract 1
month from his age of 9 months to come up with a corrected age of 8 months.
2. Download the appropriate questionnaire from our web site, or pick one up in the office. Plan to spend 15 to 20 minutes
working on the questionnaire, with your child available, as you may need to try some of the test items on your child.
3. Bring to completed questionnaire to our office for scoring!
