- North Carolina state law requires that all children entering Kindergarten have a complete physical including developmental evaluation, hearing and vision testing. A specific Kindergarten health assessment report form must be filled out. This has to be done within one calender year of starting Kindergarten.
In reference to immunizations, the state requires five DPT or DT shots, four polio vaccine doses, and two measles, mumps and rubella shots, four HiBs, and three Hepatitis B shots. - Updated: March 22, 2001
